Transplants take a tremendous emotional and physical toll on patients and their support network. Getting listed for transplant involves an intense suite of medical tests—in multiple stages—many specialists, and clinics and takes weeks and sometimes months to complete.

Transplant coordinators see many patients, and through this model, the patients' support network is at the center of their experience. In 2019, as the team was preparing for a triple organ transplant, they built a Trello board to help navigate this process and improve patient outcomes.
